How to read an AI incident report

Incident reports are becoming standard. Knowing how to read them helps consumers tell a minor bug from a serious failure.

Several public databases now track AI incidents, including the AI Incident Database maintained by the Responsible AI Collaborative and the OECD AI Incidents Monitor. These resources collect press coverage and organizational disclosures into structured records, each with a description of the system, the harm, and, where available, the response. Reading them well is a skill that consumers, journalists, and procurement officers increasingly need, because headline coverage often compresses very different kinds of failure into the same alarmed language.

A useful report usually answers four questions. What was the system trying to do, and in what deployment context. What went wrong, in terms specific enough to distinguish a user interface flaw from a core model error. Who was affected, and how were they identified. What did the operator do once the problem became known, including disclosure, rollback, or compensation. Reports that skip any of these tend to be either early rumor or deliberate public relations framing.

Severity is easier to judge when you separate harm type from harm scale. An inaccurate chatbot answer that costs one user an afternoon is different from a medical triage tool that misroutes a patient, even if both show up as a single incident entry. Likewise, a narrow bug in a widely deployed system can accumulate small harms that collectively outweigh a dramatic one-off failure. Looking for patterns across similar incidents in the same sector is usually more informative than any single story.

Editor's note: Incident literacy will become part of responsible citizenship the way financial literacy has. Consumers who can distinguish a governance failure, a model-capability failure, and a communications failure will make better choices about which services to trust. They will also hold regulators to a higher standard, since vague calls for action rarely translate into durable rules without concrete incident evidence behind them.
