I am a nurse practitioner at a clinic that serves several small communities in northern Japan. I have complicated feelings about AI in healthcare. On one hand, a decision-support tool we piloted helped me catch a medication interaction I probably would have missed on a long shift, and a triage assistant cut the time our reception spends routing calls by a real margin. These are not hypothetical benefits. They matter in a place where we are stretched thin. On the other hand, I worry about the direction of travel. Hospitals in larger cities are piloting systems that would essentially pre-stage diagnoses before a clinician sees the patient. The technology is impressive. The accountability is unclear. When a tool is wrong and a patient is harmed, whose responsibility is it? The vendor's, the institution's, mine? Current law does not answer cleanly. I would like to see our ministry engage directly with front-line clinicians, not only hospital administrators and software companies, to shape rules that reflect how care actually happens. I would like liability frameworks clarified before deployment scales. And I would like training budgets, because a tool in the hands of an undertrained user is not safer than no tool at all. I am neither for nor against. I am asking that we not rush past the hard conversations just because the demos are good.
